ACKNOWLEGEMENT
You are applying or expressing interest in a position with Concentrix CVG Corporation or one of its affiliated or related companies.
Because Concentrix CVG Corporation is a drug and alcohol-free workplace, a drug/alcohol screen may be required after you receive an offer, but prior to beginning employment, along with a background check. Employment is conditional until results are known. Drug/alcohol screens, including random-drug testing, may be required of employees. Concentrix CVG Corporation provides services 24 hours a day. Therefore, as an employee of Concentrix CVG Corporation, you may be required to work day, evening or night hours and Saturday, Sunday, holidays or overtime. You may be required to travel.
I understand that falsification, misrepresentation, or omission of requested facts will result in denial of employment or, if employed, will result in immediate dismissal. I understand and agree that, if hired, my employment will be for no definite period and may, regardless of the date of payment of wages, be terminated at any time without previous notice and with or without reason, at the will of either myself or Concentrix CVG Corporation. I agree that any oral representations made by Concentrix CVG Corporation are not binding, and that it would be unreasonable for me to rely on any such representations. I also understand and agree that no one has the authority to promise me job security or continued employment, except the President or CEO of Concentrix CVG Corporation in a written agreement signed by Concentrix CVG Corporation or me.
Concentrix CVG Corporation hires only those persons who are legally entitled to work in the United States. Every offer of employment made by Concentrix CVG Corporation is subject to the condition that the employee complete Part 1 of Form I-9 and show documents provided by law and acceptable to Concentrix CVG Corporation to prove identity and employability. Failure to meet this condition will result in an automatic revocation of the offer of employment.
Employees of Concentrix CVG are required to sign an appropriate Non-Compete and/or Non-Disclosure Agreement.
This application contains a binding jury trial waiver provision that may be enforced by me or Concentrix CVG Corporation . I understand and agree that as a condition of employment I am waiving my right to a jury trial and I request that any trial be held solely before a judge. I further understand that Concentrix CVG Corporation will waive its right to a jury trial. This waiver includes all claims and actions based on federal, state, or local law relating to the application process, my employment relationship, and the termination of that relationship. This provision and waiver do not change the at-will nature of my employment relationship in the event that I am hired.
I agree that any lawsuit relating to my employment with Concentrix CVG Corporation (or any of its subsidiaries or related entities) must be filed no more than six (6) months after the date of the employment action that is the subject of the lawsuit. I waive any statute of limitations period that is longer than six (6) months.
I further agree that I will pursue any lawsuit relating to my employment with Concentrix CVG Corporation (or any of its subsidiaries or related entities) as an individual, and will not lead, join, or serve as a member of a class or group of persons bringing such a lawsuit.
I certify that all facts contained in this application are true and complete, and I authorize Concentrix CVG Corporation to verify through background and/or criminal investigations the accuracy of the information that I have provided. I authorize former employers and educational institutions to provide information concerning me, and I release them from liability for providing any such information to Concentrix CVG Corporation. I further authorize Concentrix CVG Corporation to provide others information concerning me, and I release Concentrix CVG Corporation from liability for providing any such information.
I understand and agree that Concentrix CVG Corporation has the right to provide my name and my status as ineligible for rehire to any Concentrix CVG Corporation client for whom I do work if I am terminated by Concentrix CVG Corporation and considered ineligible for rehire by Concentrix CVG Corporation. I also understand and agree that my employment may be contingent upon my being given software access by Concentrix CVG Corporation clients and access to any client’s software or considered otherwise ineligible to work on any Concentrix CVG Corporation client account, my employment is subject to immediate termination.
I understand that if I become employed by Concentrix CVG Corporation I may be asked to sign a document attesting to my consent to all of the conditions in this Acknowledgment.
Apart from the foregoing conditions, I acknowledge that no other Concentrix CVG Corporation documents, statements of policy, nor the maintenance of supervisory or other policies or procedures shall be construed as constituting a promise from or contract of any kind with Concentrix CVG Corporation, either express or implied, regarding any of the matters addressed in such documents or policies.
On any of the telephone numbers or e-mail addresses provided by me to Concentrix CVG Corporation I consent to receive communication in any manner, including but not limited to, automated emails, voice mails, written statements, text messages, auto-dialed calls and pre-recorded messages, which could result in charges to me.
